Patient Rights
- You have the right to receive healthcare on the basis of clinical need, regardless of income.
- You have the right to be registered with a practice. If a practice is unwilling to take you on their list, Primary Care Services will arrange for your registration. You are entitled to change your practice at any time and you do not have to give a reason for your decision.
- You have the right to accept or refuse treatment. No care or treatment will be given without your informed consent.
- You can refuse to be examined or treated in the presence of medical students.
- You can refuse to be involved in research trials. If you do not agree to be involved in research trials, you can withdraw at any time.
- You have the right to equal treatment regardless of race, gender, age or disability.
- You have the right to information on practices in your area and the services they provide. Please read our practice leaflet as this will help you get the best out of the services we offer.
- You have the right to make a complaint. We will provide you with information on how to complain or make suggestions about the care we offer. We welcome any comments you may have.
- You have the right to confidentiality. Personal information about your health is confidential and will only be disclosed to those who need that information to provide you with effective treatment.
- You have the right to see your health records from the day you were born, where they exist.
- You have the right to have reasonable access to high quality service and facilities.
- You have the right to be informed on what is wrong with you and the treatment options available. Please ask questions and you will be given truthful, clear answers. In the interests of your health, it is important you understand so please ask questions if you are unsure of anything.
- You have the right to request a second opinion on your diagnosis or treatment.
- You can ask to have someone with you (friend, relative, interpreter) at any time. You may find this beneficial, particularly if you are asking questions or need some moral support.
Patient Responsibilities
- Keep appointments and try to arrive on time. If you are unable to keep an appointment, please let us know as soon as possible as there is always great pressure on our appointment system.
- If you move house or change any of your contact details, please let us know.
- Treat all staff in a reasonable and courteous manner.
- Use emergency services in a responsible manner. Please use the out-of-hours services for emergencies only.
- Take care with medicines as they are for one person only and should not be shared. Keep them safely away form children and in the original container. Any unwanted medicines can be taken to a chemist for safe disposal.
