New Patient Registration

We welcome new patients from within our boundary


Registration forms can be obtained from reception.

Patients wishing to register with us will be expected to attend a new patient medical check. 

You will be asked to provide ID and proof of your address.  You will also be asked to provide evidence of intended length of stay if you are from overseas. 

Please note that you will be registered with the practice and not an individual GP, although patients are welcome to consult with the doctor of their choice where appropriate.

To avoid delay and having to be kept waiting, please avoid registering with us first thing in the morning and between 12.30 and 1.30pm. 


Change of Personal Details

If you change your name, address or telephone number, please notify us immediately so that we can amend your records.  If your new address is outwith the practice area, we will have to ask you to register with another practice.
